Insurance coverage for ophthalmology services and eye surgery can vary widely, depending on the plan. Usually elective surgery such as LASIK is not covered by insurance. Cataract surgery is usually covered, but upgrading a standard intraocular lens (IOL) to a premium IOL will require an out-of-pocket cost.
